1. On your mobile device, install the Sophos Authenticator app from the Apple App Store or Google Play Store.
2. Log into connection.bayviewphysicians.com to display your personalized QR code.
3. Launch the Sophos Authenticator app on your phone, and scan the QR code on your screen to synchronize your phone and the website.
4. Enter the code from the Authenticator app into the PIN field on this form. The code will change every 30 seconds, so make sure you log in before the PIN expires.

If you previously set up Sophos Authenticator on another phone, you will need to contact the Help Desk to set it up on your new device.
